Effective
The provider's assessment and care planning processes enabled and empowered staff to deliver effective, good quality care and support to people. The provider worked closely with local authority commissioners to identify improvements to the assessment of needs for people discharged from hospital to be supported in their own home.

Assessing needs
People's feedback about the service was consistently good. People were satisfied their care and support met their needs and supported their wellbeing. One person said, "Carers also support my emotional wellbeing by having a chat with me. I feel listened to and my concerns are addressed." People's relatives appreciated that the person could be discharged from hospital with a package of care to support them in their own home.
The provider had effective processes for the assessment of needs, care planning, risk assessments and care reviews. Records showed these were completed thoroughly and professionally.
Staff told us the online care planning system gave them the information they needed to support people according to their needs and preferences. It included information about people's assessed needs, any identified risks to their safety and wellbeing, and instructions about individual support tasks. Support was designed to help people to achieve the desired outcome from their care. The registered manager had worked with other involved agencies to improve the assessment process for people discharged from hospital. This followed an incident where the provider's staff had not received all the information about a person's needs to enable a comprehensive care plan to be developed.
