People’s nutrition and hydration needs were not always met as staff were not always available to support people where needed. Staff were able to explain why it was important to monitor intake and the risks associated to people with poor food and fluid intake. Where records showed people had insufficient amount to drink, action had not been taken to address this and seek further advice from the appropriate healthcare professionals. The provider and registered manager had not considered or implemented evidenced based care and support for people living with dementia.

People’s nutrition and hydration needs were not always met as staff were not always available to support people where needed. Records showed some people at risk of dehydration had a low fluid intake and action had not been taken to address this and seek further advice. A relative told us, “I have seen them [people] sitting looking at food getting cold.” Another relative told us, “[Person] does get a good choice of food, but he does not drink enough and gets dehydrated. I have been up there when the staff have brought him cups of tea but he often leaves it and doesn't drink it.” People living with dementia were not supported to engage in activities they liked doing such as crafts, singing, dancing and gardening. People were not encouraged to spend time reminiscing and sharing photos, memories, favourite music, films or books. This was a missed opportunity to support people and reduce distress, anxiety, and isolation.

Staff told us they monitored people’s food and fluid intake. Staff were able to explain why it was important to monitor intake and the risks associated to people with poor food and fluid intake. A staff member said, “We regularly offer food and fluids and encourage residents to stay hydrated.” However, following our observations made during the assessment and the feedback received from people’s relatives, we were not assured staff always supported people with their food and fluid, as needed. Staff told us they would contact a person’s GP and dietician for advice and support for a person loosing weight.

The provider and registered manager did not always consider evidence-based guidance when planning and delivering care to people. There was no oversight of people’s food and fluid intake, for people at risk of malnutrition or dehydration. This meant where records showed people had insufficient amount to drink, action had not been taken to address this and seek further advice from the appropriate healthcare professionals. Records and oversight of this had improved in the time frame between our first and second site visit for this assessment. This needed to be embedded and sustained into practice. The provider and registered manager had provided staff with training in dementia awareness, however, they had not considered or implemented evidenced based care and support for people living with dementia.
