Winsford Grange is a 'care home'. People in care homes receive accommodation and nursing and/or personal care as a single package under one contractual agreement dependent on their registration with us. Winsford Grange is a care home with nursing care. CQC regulates both the premises, and the care provided, and both were looked at during this assessment. This provider is required to have a registered manager to oversee the delivery of regulated activities at this location. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Registered managers and providers are legally responsible for how the service is run, for the quality and safety of the care provided and compliance with regulations. At the time of our assessment Winsford Grange had a registered manager in place. This was an unannounced, responsive assessment, prompted by emerging risk. During the assessment, we assessed 34 quality statements and identified 9 breaches of Health and Social Care Act 2008 (Regulated Activities) including person centred care, dignity and respect, need for consent, safe care and treatment, safeguarding service users from abuse and improper treatment, meeting nutritional and hydration needs, premises and equipment, good governance and staffing. The previous rating for this service was requires improvement published 15 February 2023. During the previous report the service was in breach of regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. We identified the service remained in breach of regulation 17.

People's experience of the service

Most people felt safe within the service however, some family member raised concerns regarding the safety of their relatives. One relative was concerned over the security of the premises due to the unit not being secured. People did not always receive their medication when required, one person told us, “A couple of times [staff] have said they are waiting on the pharmacy I've been left howling in pain, I have to wait a long time for pain relief.” There was mixed feedback on the support provided by the service. All people spoken to reported there was not enough staff, this sometimes impacted on them receiving timely support, relatives were concerned their loved ones were still in bed late on in the morning. Some people told us they were not provided with a choice at mealtimes and the food was regularly cold. Relatives told us they could raise a concern however it was not always dealt with. People told us they were not always supported to access health professionals when required. One person had been waiting for their glasses to be fixed whilst another person was waiting to see a dentist for dentures. People told us they had not been involved in their care planning and not everyone was aware they had one in place. People told us they could not access the gardens due to how overgrown they were, or the doors were locked. Some people told us they were not allowed out without a carer despite them being independently mobile.
