We reviewed the care provision, integration and continuity quality statement for the responsive key question. Care provision, integration and continuity: families told us there was continuity of care and, communication between the service and other organisations was beneficial to their children’s care.

Care provision, Integration and continuity

Score: 3

Most families were happy with the care provided by Jessie May Trust and told us they benefitted from the continuity of care the service provided at each visit and during times their children were admitted to hospital. Parents told us staff were responsive to their children’s needs as they changed.

Staff and families told us the working relationships and communication between Jessie May Trust and other organisations involved with children’s care helped to provide safe care for children and their families’ distinct needs.

The Starling project, a collaborative End of Life care pilot between providers to facilitate care of children who wished to die at home demonstrated this approach. Collaboration between providers benefited patients and families.

Offers of care were calculated using a scoring system. Some families felt they had enough visits. However, some families receiving holiday-only visits told us this was not adequate for their needs. Some families and staff told us they felt the scoring system used did not always seem fair. However, we were told a more inclusive tool to assess children for their offer of care to ensure improved equity going forward was due to be implemented shortly.
