The inspection
We carried out this inspection under Section 60 of the Health and Social Care Act 2008 (the Act) as part of our regulatory functions. We checked whether the provider was meeting the legal requirements and regulations associated with the Act. We looked at the overall quality of the service and provided a rating for the service under the Health and Social Care Act 2008.
Inspection team
This inspection was conducted by one inspector, an assistant inspector, a medicines inspector and an Expert by Experience. An Expert by Experience is a person who has personal experience of using or caring for someone who uses this type of care service.
Service and service type
Mount Olivet is a ‘care home’. People in care homes receive accommodation and nursing and/or personal care as a single package under one contractual agreement dependent on their registration with us. Mount Olivet is a care home with nursing care. CQC regulates both the premises and the care provided, and both were looked at during this inspection.
Registered Manager
This provider is required to have a registered manager to oversee the delivery of regulated activities at this location. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Registered managers and providers are legally responsible for how the service is run, for the quality and safety of the care provided and compliance with regulations.
At the time of our inspection there was a registered manager in post.

What we did before the inspection
We reviewed information we had received about the service since the last inspection. We sought feedback from the local authority and professionals who work with the service. We used the information the provider sent us in the provider information return (PIR). This is information providers are required to send us annually with key information about their service, what they do well, and improvements they plan to make. We used all this information to plan our inspection.
During the inspection
We spoke with 9 people who lived at the service and 5 visitors. We spoke with 9 staff members; this included the registered manager, administrator, care staff and registered nurses. We received feedback from 3 health and social care professionals. We reviewed the records relating to the running of the service. These included recruitment records, accidents and incidents and quality assurance audits. We looked at medicine administration charts, 11 care plans and risk assessments along with other relevant documentation to support our findings.

About the service
Mount Olivet is a residential care home providing personal and nursing care to up to 30 people. The service provides support to older people. At the time of our inspection there were 22 people using the service.
People’s experience of using this service and what we found
Risks to people's health, safety and welfare had been identified and guidance was in place to manage risk. However, risks to people were not always managed effectively as monitoring charts were not being completed consistently in relation to skin care and nutrition.
People did not always receive their topical medicines as prescribed for them. Topical administration medicines records were in place, but these were not always completed to demonstrate creams had been applied. Sufficient guidance was not in place for people prescribed oxygen.
Quality assurance and governance systems in place to assess, monitor, and improve the quality and safety of the services provided had not identified all of the issues we found during this inspection.
During the inspection we observed there were enough staff to care for people effectively and safely. However, people and their relatives told us they sometimes had to wait for care. We made a recommendation about this.
People told us staff were kind, caring and treated them with dignity and respect. People were encouraged and supported to remain as independent as possible and were supported to make decisions about their care.
The registered manager promoted a positive, person-centred culture within the service. People were very positive about the registered manager and told us they were approachable and very ‘hands on’.
People told us they felt safe. Staff had received safeguarding training and recognised different types of abuse and how to report it.
Recruitment processes ensured that, as far as possible, only suitable staff were employed to look after people.
People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.
People were protected from the risk and spread of infection. Staff were using personal protective equipment (PPE) in accordance with the government guidance.
People and their relatives were asked to provide feedback about the service and the provider sent out yearly questionnaires to people and relatives to gather their views.

Rating at last inspection
The last rating for this service was outstanding (published 16 January 2018).
Why we inspected
This inspection was prompted by a review of the information we held about this service.
We looked at infection prevention and control measures under the Safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively.
Enforcement and Recommendations
We have identified breaches in relation to the management of risk and governance at this inspection. We have also made a recommendation about how the service reviews its staffing arrangements and how staff are deployed to ensure people receive care that meets their needs and protect their safety.
