Delivering evidence-based care and treatment
People had individual care plans which provided staff with the information they required to meet people's needs. People’s needs were assessed before they received a service and regularly thereafter.
People were supported with comprehensive needs assessments and leaders ensured care plans reflected people’s needs. Best practice was followed when carrying out assessments and this included involving people in them.

Supporting people to live healthier lives
People’s healthcare needs were regularly reviewed. Where monitoring was required for specific needs this was carried out. For example, people’s weights, blood pressure, blood sugar levels and skin integrity were checked and recorded.
The manager ensured people had access to healthcare services. These included GP’s, opticians, and dentists. Where required referrals were made to healthcare professionals for assessment and guidance.
People told us they had access to healthcare professionals such as the GP and the service supported them with this access.
