During an inspection looking at part of the service

On 15 January 2013 we carried out an inspection at Grosvenor House Care Home. This inspection was to assess whether a compliance action we set following our previous inspection in August 2012 had been met. At the time of our inspection twenty three people were using the service.

In August 2012 we found that the provider was not meeting the essential standards relating to the management of medicines.

During this inspection we talked with the registered manager and the nurse on duty while assessing the improvements made.

We found that since our previous inspection improvements had been made. The records and audits of medication and creams showed that medicines were administered or applied as prescribed.

In this report the name of one registered manager appears who was not in post and not managing the regulatory activities at this location at the time of the inspection. The name appears because they were still a Registered Manager on our register at the time.

During a routine inspection

This inspection was unannounced which meant no one living at or working at Grosvenor House knew that we were coming. The manager was on annual leave on the day of this inspection.

During our inspection we spent time observing four people who used the service while they were in the dining room. We spent time speaking with these people as well as other people while they were in the lounge areas. We also spoke to care staff, the nurse on duty and the regional manager.

While at Grosvenor House we saw that care workers were being courteous and respectful towards people who used the service ensuring that people's dignity was upheld.

Care plans, risk assessments and other records were generally in place to give staff guidance and direction about the level of care required to meet individual needs.

Information on safeguarding was available for people in the home including members of staff. Staff training records were available and it was evident that they had an understanding of their responsibilities in the event of them becoming aware or suspecting actual or potential abusive events.

We found communal bathrooms and toilets to be suitably clean with hand washing facilities provided.

We found that improvements were needed in the way the home managed people's medication.

We found that staff had undertaken training to support the people they were caring for.

Systems were in place to assess and monitor the quality of care provided and as a means of identifying any improvements that were needed.

During an inspection in response to concerns

Many of the people residing at Grosvenor House were not able to tell us much about their experience at the home due to their condition but most of those who could were happy with the care provided. Nobody told us of any concerns that they have.

We observed staff while we were at the home and found them to be busy throughout the day. Although they were kind and caring people often had to wait to have their personal care needs met.

We became aware of issues regarding caring for people's oral hygiene. We saw shortfalls regarding record keeping in areas such as fluid intake whereby they suggested that people were not receiving adequate drinks or having their blood sugar levels checked.

The majority of staff have received training in safeguarding of vulnerable adults. We did however become aware of two incidents whereby there was no recorded consideration about making a referral to the local authority under safeguarding.

We found that improvements are needed in some areas regarding cleanliness of the home and decontamination of some equipment. We were assured that improvements will be made.

Improvement is needed with the management of medication. The application of creams is not recorded and some medication was stored at too high a temperature.
