Score: 3

People told us staff supported them to access health appointments as needed. One person said, “I see my GP sometimes.” The person told us they also saw their optician and staff had helped them to manage a health concern they had recently.

Staff understood people’s needs and how to support them to access health and social care support if needed. Staff told us they had received training on people’s unique health conditions. This meant they could recognise changes in the person’s health then take appropriate action. This was evident when describing how they would support a person who had an epileptic seizure.

There was a clear policy on equality and diversity. This policy encouraged staff to treat people well and provide people with equal opportunities regardless of their protected characteristics. People’s unique needs and identity were recorded in their care plans. Staff were given guidance on how to support the person’s needs to ensure positive outcomes. These were reviewed with people to ensure they were happy with the care and support provided. The manager was aware of the Accessible Information Standard. Since 2016 all organisations that provide publicly funded adult social care are legally required to follow the Accessible Information Standard. The Accessible Information Standard tells organisations what they have to do to help ensure people with a disability or sensory loss, and in some circumstances, their carers, get information in a way they can understand it. It also says that people should get the support they need in relation to communication. There were examples of ‘Easy Read’ guidance and policies and procedures for people, should they need them. Records were kept of when health professionals visited, and what advice they gave people. Staff used this information to ensure this gave people positive outcomes. We did note for one person a health professional had given instructions on how the person’s diet should be restricted. This did not result in an amended care plan. However, when we discussed this person’s needs with kitchen staff and care staff, they all understood how to support this person safely. We raised this with the manager who told us they amend this to reduce the risk to the person’s health. Staff monitored people’s daily wellbeing and recorded when their health had changed. This allowed staff to communicate with the person about their ill health, identify if external health or social care support was needed, and arrange this promptly.
