Condition of this registration relating to carrying out this regulated activity
Within 28 days of the condition taking effect, the Registered Provider must:
Have effective governance systems in place to ensure they carry out effective audits on;
a. Service users care plans and risk assessments, to ensure information is up to date and accurate.
b. Service users skin integrity monitoring.
i. To ensure body maps are implemented and updated when service users skin condition changes.
ii. Staff are clearly recording the support provided to preserve skin integrity and manage pressure sores; this must include repositioning where required and the condition of the skin, as per your own policy.
c. Service users call monitoring.
i. To ensure calls are taking place at the times specified in the service users care plan.
ii. To ensure calls are taking place for the correct length of time.
iii. Ensuring calls are attended by the correct number of staff for the duration of the call.
iv. To ensure staff are recording the care notes at the time support is provided and not retrospectively.
d. Medication administration records.
i. A record of how to support each service users to take their medication is clearly documented in care plans and risk assessments.
ii. All prescribed medicines including topical creams are clearly recorded on the electronic Medication Administration Records (MAR)record and detail how often to be given, when to be given
r where to be applied.
iii. Ensure clear written protocols for ‘As required’ medicines are in place to guide staff on how to safely administer such medicines.
e. Staff schedules. You must implement an effective system to ensure staff are not logging in to multiple calls at the same time or overlapping calls, so you have a clear and accurate record of when staff are in
attendance of calls.
f. Staff files. Valid insurance documents for all subcontract staff should be held on staff files as per your own agreement.
Within 14 days after this condition takes effect and then every first Monday of each calendar month thereafter, the Registered Provider must send to the Commission a written report outlining the findings of these audits. The report should contain details of the timescales for improvements, who will be responsible for them and what the quality assurance arrangements will be going forward to ensure that the agreed actions are completed.
