During this assessment we saw some improvements in the way the practice was led and managed although some improvements were required for safe care. We found improvements in the systems and processes for safeguarding and information workflow as well as developments in communication and management systems. We found two breaches of regulations in relation to Safe care and treatment and Good governance. We found concerns around the management of people’s medicines, care and treatment for people with long-term conditions, which has resulted in a breach of regulation 12 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. We found a lack of effective governance systems and processes to assess, monitor and improve the quality and safety of the service, including the risk management, emergency equipment and medicines monitoring of cold chain, staff recruitment files and the accuracy of patient records which resulted in a breach of regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
