People were supported to have their needs assessed and a care plan developed and reviewed regularly. Care plans contained clear guidance for staff on how to support people effectively and with the latest best practice documented, which explained and guided staff in delivering person centred care to support peoples need and wishes.

Assessing needs

People we spoke with told us Premier Community assessed their needs prior to the care starting and continued to regularly review their needs with them. One person said, “A manager came out recently to review my care plan with me, there were no changes but it was good to discuss everything and meet the management.”

Staff told us they were supported and encouraged to feedback changes to people needs or gaps in care planning. One staff member said, “I have a named lead care co-ordinator that I can go to, so if someone’s needs have changed, I can let them know and they act really quickly, usually the same day the care plan is updated.” Another staff member said, “We have a message app on our phone so if there are changes to someone’s care I know instantly, it’s a really proactive system.”

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, well-being and communication needs with them. Where people needed staff to support them with specific high risk needs, these had been assessed and documented. For example, where someone had a catheter in-situ, risk assessments were in place to guide staff on how to monitor for the risk of infection. The management team regularly sought feedback from people to ensure their needs were met this was done by various methods such as in home visits, telephone calls and survey results.

Delivering evidence-based care and treatment

People we spoke with did not or were not able to comment on evidence-based care and treatment. However, our assessment showed people received care in line with current best practice and guidance for conditions such as diabetes and catheter care.

Staff told us the provider and management team were responsive to their training needs for delivering evidence-based care and treatment. One Staff member said, “The training team are great, if I need extra training they assign the course to me in the relevant areas and if it’s not available, they create something bespoke.” We saw evidence of this in the form of video tutorials involving people with their stoma care which gave person specific training and insight in people needs and best practice.

Care plans contained condition specific information for staff to be able to support people with their health conditions. There were clear processes in place for staff to seek assistance or guidance in an emergency, for example if some experience a choking incident or epileptic seizure. We spoke with the registered manager who was going to review adding further information about where staff could obtain condition specify guidance for upskilling and early detection of declining health needs.
