Fleetwood Same Day Health Centre (Fleetwood Health and Wellbeing Centre, Dock Street, Fleetwood, Lancashire, FY7 6HP) is a registered location under the provider PDS Medical Limited and delivers urgent care services to the Fleetwood population. The service is part of the NHS Fylde and Wyre Clinical Commissioning Group (CCG). The provider is registered to carry out the regulated activities of diagnostic and screening procedures, transport services, triage and medical advice provided remotely as well as treatment of disease, disorder or injury at this location. PDS Medical Limited are working in collaboration with other local urgent care providers to align the service provision at Fleetwood Same Day Health Centre with other local sites as part of the Fylde Coast Integrated Urgent Care Service. The providers share managerial oversight of the urgent care provision across the system in order to better manage demand and resources to ensure patient needs are met.
The Same Day Health Centre service is situated within a purpose built primary care centre. The premises is close to the town centre of Fleetwood and is serviced by good public transport links, with ample parking available. The building is shared by a GP practice, pharmacy, x-ray service and mental health service.
The centre provides assessment, diagnosis, treatment and advice to patients presenting with minor illness and minor injuries and is accessible to local and temporary residents and visitors to Fleetwood. The Same Day Health Centre is open between 8am and 8pm, seven days a week, all year round including Christmas Day. Patients can self present without an appointment, irrespective of whether they are registered with a GP, with any primary care need. The service also works closely with the NHS 111 service and pre-bookable appointments are available to patients accessing the service via this pathway. Patients will be assessed and treated or referred /signposted to other services if required.
In addition, the provider also delivers extended access sessions from the centre between 6.30 and 9.00pm on a Monday and Tuesday, between 6.30 and 8pm Wednesday to Friday, and also between 8am and 2pm at weekends. Extended access sessions are also offered from a branch site at Freckleton Health Centre (Douglas Drive, Freckleton, Preston PR4 1RY) between 6.30 and 9.30pm Monday, Tuesday and Thursday, and between 8am and 2pm at weekends. Appointments are offered as part of these sessions for patients registered with local GP practices who are unable to attend their usual surgery during normal opening hours.
The provider delivers a nurse-led wound care service from the Same Day Health Centre, with pre-bookable appointments available between 8am and 4pm, Monday to Friday. This service is also offered from two other local branch sites (St Annes Primary Care Centre, Durham Avenue, St Annes, FY8 2EP and Kirkham Health Centre, Moor Street, Kirkham, PR4 2DL).
The urgent treatment centre provision is staffed by a team of GPs, advanced nurse practitioners, non-medical prescribers, practice nurses and health care assistants. The clinical team are supported by an operational lead and team of other non-clinical personnel including receptionists and administration staff. The staff at the centre are also supported by a broader management structure within the provider organisation and those other providers in collaboration as part of the integrated urgent care system across the Fylde coast.
The provider ensures a GP is on site throughout the location’s operational hours of 8am until 8pm each day.
