Assessing needs
Relatives we spoke to felt staff knew people's needs well. Comments included: “They seem to know [person] better than me now. They understand him and help him settle again.” And: “She has been there a long time now and staff do know her well. Although she doesn’t talk, she makes her feelings known.”
Staff felt people's needs were assessed well and that care and support plans reflected people’s needs. One staff member explained how care plans were reviewed as an ongoing process; they told us: “Things change so much anyway because of [people’s] needs, it is a constant review. We can add to care plans. We talk about this a lot as a team.”
We observed documents about people’s health and wellbeing were available in easy read formats. This meant documents were available to help people understand their care and treatment. We saw people had detailed reviews of their care and support and saw social workers and people’s relatives were invited to these reviews.
