Condition of this registration relating to carrying out this regulated activity
The registered provider must ensure that care plans for each service user have been developed following an individualised risk assessment. Risk assessments must consistently consider risk to the service users personal health, safety and welfare and any risk they present to other service users or staff.
Following any incident where staff have been involved in restraining a service user or any other person the registered provider must provide the Care Quality Commission with a report detailing the following:
Details of the incident which resulted in restraint becoming necessary;
Details of de-escalation strategies used prior to restraint becoming necessary;
Confirmation that the individuals risk assessment and care plan has been updated;
Confirmation that the service users liberty and human rights have not been infringed by the action taken.
The registered provider must ensure that any service user who may be a risk to themselves or others if they left the service without notifying staff has a detailed risk assessment and care plan in place. This must direct staff as to the action to be taken if the service user leaves the service without staff awareness or fails to return at the expected time. Following any incident where a service user is reported as missing from the service, the registered provider must provide the Care Quality Commission with a report detailing the following;
Details of the incident, including the time that the individual went missing and actions taken by staff in response to this.
Confirmation that risk assessments and care plans have been updated to reflect the risk posed to the service users and others when missing from the service.
