Background to this inspection

Updated 29 October 2018

Sandringham Practice is situated within the NHS City and Hackney Clinical Commissioning Group. The practice holds a General Medical Services contract (a GMS contract is a contract between general practices and NHS England for delivering primary care services to local communities).

The practice is located at 1A Madinah Road, London, E8 1PG and its website can be found at: www.sandringhampractice.nhs.uk.

Sandringham Practice registered with the Care Quality Commission on 17 October 2017 to carry on the regulated activities of Treatment of disease, disorder or injury; Diagnostic and screening procedures; Maternity and midwifery services. It has a patient list of approximately 3,750 patients.

The staff team at the practice includes one male part-time GP principal, a female full-time salaried GP a male full-time salaried GP, and a female part-time regular locum GP. Between them the GPs work the equivalent of two and a half full-time GPs. The clinical team is completed by a female full-time practice nurse and a female full-time health care assistant.

The non-clinical staff consist of a full-time practice manager, a part-time business manager, four full-time administrative and reception staff and a part-time note summariser/clinical coder.

The practice is open:

Monday, Tuesday, Thursday and Friday 8.00am - 6.30pm.

Wednesday8.00am to 8.00pm

Saturday 9.00am – 1.00pm

The practice runs clinics for the benefit of working patients on Wednesday evenings until 8.00pm, and Saturdays between 9.00am – 1.00pm. Outside of these hours patients can access appointments at one of five local GP Hub locations which offer appointments: 4.00pm-8.00pm on weekdays and 8.00am-8.00pm on weekends.

To assist patients in accessing the practice there is an online booking system. Urgent appointments and home visits are available each day and GPs also provide telephone consultations for patients. During evenings and weekends, when the practice is closed, patients are directed to an Out of Hours service delivered by another provider.

Sandringham Practice serves a practice population with a deprivation score that is higher than the England average. For example, 38% of children live in income deprived households, compared to a local average of 33%, and the national average of 20%.

Overall inspection

Good

Updated 29 October 2018

This practice is rated as Good overall. (It has not previously been inspected)

The key questions at this inspection are rated as:

Are services safe? – Good

Are services effective? – Good

Are services caring? – Good

Are services responsive? – Good

Are services well-led? - Good

We carried out an announced comprehensive inspection at Sandringham Practice on 20 September 2018 as part of our inspection programme.

At this inspection we found:

  • Published practice performance data generally pre-dates the date on which (17 October 2017) the new provider took over responsibility for running the practice. Accordingly, this has been reflected in our report. For example, for the data collection period 1 April 2016 – 31 March 2017 the practice had screened 68% of eligible patients for cervical cancer, which was below the national screening target of 80%. For the period 1 April 2017 – 31 March 2018 the practice showed us with its unpublished, unverified, data which showed that it had screened 81% of eligible patients.
  • Clinical waste was appropriately stored, but the practice did not label bags. During our inspection the practice labelled the clinical waste bags currently in use and introduced an amended procedure to ensure that this would continue to be done.
  • We saw no evidence of discrimination when making care and treatment decisions.
  • The practice had systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
  • The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
  • Staff involved and treated patients with compassion, kindness, dignity and respect.
  • Patients found the appointment system easy to use and reported that they were able to access care when they needed it.
  • There was a strong focus on continuous learning and improvement at all levels of the organisation.

The areas where the provider should make improvements are:

  • Review the security arrangements for prescription computer paper.
  • Review its patient participation group and work to hold regular meetings and encourage new members to join.
  • Review and continue to work to improve uptake of its child immunisation programme for the benefit of that patient population.
  • Review and continue to work to improve uptake of its cancer screening programmes for the benefit of that patient population
  • Review and ensure that all outstanding actions identified in the Infection Prevention and Control audit and fire risk assessments are addressed.
