There were significant breaches of 7 regulations at the assessment of this service, in relation to safe care and treatment, safeguarding service users from abuse and improper treatment, person centred care, dignity and respect, premises and the governance of the service at Welham House.

Accommodation for persons who require nursing or personal care

Condition of this registration relating to carrying out this regulated activity
The registered provider must seek support from an independent professional who has in-depth knowledge on the NICE Guidance – Managing medicines in care homes, on the safe administrations of medicines in Welham House. The registered provider must inform the Care Quality Commission in writing by 4pm on 2 October 2023 with details of who has been appointed.
Staff must receive training in the safe administration of medicines and have their competencies checked by a suitable qualified independent professional by 6 October 2023.
The registered provider must seek support from a positive behaviour support professional to develop clear and robust behaviour support plans and behaviour risk assessments for Service Users. Support plans and risk assessments must include clear details of the following:
a. evidence of learning from recent incidents,
b. behavioural triggers and strategies for support,
c. escalation plan / crisis intervention plan,
d. the use of ‘as required medication’ to manage behaviour,
e. where appropriate plans must contain detailed, personalised information about any prescribed physical interventions safe to use with the service user.
Support plans and risk assessments for all service users must be emailed to the Commission within 5 calendar days of imposition of this condition.
The registered provider must send to the CQC an overview of all behavioural incidents at Welham House, including: -
a. and this must include details of the incident,
b. immediate action taken by staff,
c. clear details of any restraint used, longer-term actions and learning
This must be sent to the Commission on the first Monday of each calendar month.
The registered provider must review all service users care plans to identify any risks to the public. Risk assessments must be put into place to mitigate those risks. Support plans and risk assessments for all service users identified as a risk to the public must be emailed to the Commission by 6 October 2023.
The registered provider must follow the risk assessments in service users’ care plans to keep the public safe from harm. This must include but is not limited to:
a. ensuring service users assessed as needing one to one support hours are identified and care delivered by the appropriate number of staff.
b. That service users are appropriately monitored/ supervised when out in the community and within areas of the service from which they could engage with the public.
c. That service users are appropriately monitored/ supervised within the service, this includes access to electronic devices including television is monitored appropriately.
The registered provider must send to the CQC an overview of the monitoring completed to keep the public safe from harm both at Welham House and in the community. This must include: -
a. details of when service users, who are a known risk, leave and return to the home,
b. spend time in communal areas both in the home and in the garden where they can interact with members of the public
c. and any monitoring of their environment and use of electronic devices.
This must be provided to the Commission on the first Monday of each calendar month.
Terms of this registration relating to carrying out this regulated activity
The Registered Provider must not provide nursing care under accommodation for persons who require nursing or personal care at Welham House.
The Registered Provider must only accommodate a maximum of 14 service users at Welham House.
