This service scored 75 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.

Assessing needs

Score: 3

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s care plans were personalised and included people’s wishes and preferences.

Delivering evidence-based care and treatment

Score: 3

The service planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. For example, people’s care plans described the impact of their mental health on their day to day lives and detailed the support they needed in line with best practice.

How staff, teams and services work together

Score: 3

The service worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. The service worked with external professionals when people moved into the service, ensuring a detailed pre-assessment was completed.

Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. Care plans included detailed information about people’s healthcare needs and how these were met. Relatives provided numerous examples of how people had achieved improved quality of life and well being through staff support.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. For example, a person was supported to access regular testing and monitoring to ensure their medicine remained effective in meeting their needs.

The service told people about their rights around consent and respected these when delivering person-centred care and treatment. People’s care plans included detailed information about how people made decisions and choices. Where people lacked capacity around specific decisions, the service had assessed people’s capacity in line with the Mental Capacity Act 2005. A person told us, “I choose how I spend my time and what I do. Staff have always respected this and supported me with my choices.”
