There were clear governance, management and accountability arrangements. Staff understood their role and responsibilities. Leaders and managers could account for the actions, behaviours and performance of staff. Staff told us managers and leaders were visible and approachable.

Data or notifications were consistently submitted to external organisations as required. There were arrangements for the availability, integrity and confidentiality of data, records and data management systems. However, information was not always used effectively to monitor and improve the quality of care. We reviewed a number of clinical audits (pregabalin, gabapentin, lithium, antibiotics). We did not find a summary of findings, learning outcomes or discussion of results. This meant it was unknown whether there had been any learning or impact as a result of the audits. An audit should include a summary or discussion of results, list of potential improvements to be made and recommendations of how to implement change, and then be re-audited to see if there is any change and what impact the audit had.
