Our rating for this key question remains good. The provider took steps to ensure the effectiveness and positive outcomes of people’s care and support by assessing and reviewing their care and communication needs with them and, where appropriate, their relatives.

Assessing needs

People and their relatives felt involved in the planning and delivery of their care and support. They were confident staff understood their individual needs and had the support and training they needed to meet these. They spoke about some of the ways in which staff met their needs, including support with maintaining their independence or managing specific health conditions. They felt freely able to communicate their wishes, needs and preferences to staff and management and commented on the good communication with the service.

Senior staff and management were able to explain to us how they ensured people and, where appropriate, their relatives were central to the assessment, planning and review of their care needs and preferences. They understood the need to adopt a person-centred approach in order to tailor the planning and delivery of care to people’s individual needs, and the importance of working effectively and in a coordinated manner with other health and social care professionals involved.

The provider had robust systems, processes and tools in place, designed to ensure a comprehensive assessment and regular reviews of people’s individual care needs and preferences to achieve positive outcomes for them. This included consideration of people’s communication needs, mobility, pressure care, physical and mental health, nutrition and hydration, known likes and dislikes, valued relationships and their home environment. People’s individual needs, wishes and preferences were reviewed with them annually, or sooner if required. The provider’s electronic care management system was designed to ensure staff had continual access to accurate, up-to-date and complete information and guidance on people’s needs, preferences and risks to promote safe and person-centred care. People’s assessments and care plans reflected a strengths-based approach, highlighting their skills, abilities and goals.
