Effective
We have not rated the effective key question as it was not rated at our last inspection in July 2015 and we did not look at all the quality statements for the effective key question during this assessment. Staff followed up-to-date policies to plan and deliver high quality care according to best practice and national guidance. Staff had access to the hospital’s policies and procedures via their computer system. We saw improvements had been made following post-operative blood clots. Patients were given advice about risks of blood clots in veins during their pre-assessment. The service had also introduced a risk assessment to be completed for all patients before their surgery. Staff had mostly fully completed assessments for venous thromboembolism (VTE), a term referring to blood clots in the veins. However, an audit of these assessments had shown that clinicians were not always signing the form. This was highlighted to nursing staff at team meetings however, there was no evidence that this was highlighted to all clinical staff.

Delivering evidence-based care and treatment
The service recieved NICE guidance updates through a reporting system which was set corporately by Nuffield Health. This meant that the service leaders were aware of new guidance suggested to meet patient care and safety needs. The hospital used evidence-based guidance and quality standards to inform the effective delivery of care and treatment. For example, staff in the pre-operative assessment clinic assessed patients in accordance with the National Institute for Health and Care Excellence NG45 ‘Routine pre-operative tests for elective surgery’. The service participated in relevant local and national audits such as the audit of surgical site infections which were based on national guidance, standards and legislation, including NICE guidance, the Royal College of Surgeons, and the Health and Safety Executive. Hospital and corporate policies reflected national and local guidelines. There was a specialist services monthly newsletter, specific for the surgical service, that reviewed new guidance to be implemented and new hospital policies It also referenced current national guidelines and relevant evidence. We reviewed minutes from the medical advisory committee (MAC) which showed consultants reviewed patient outcomes, patient experience of treatment and recovery and efficient use of resources.
Staff we spoke with had access to the hospital’s policies and procedures via their computer system. The ward staff told us they used care bundles in line with the NICE guidance. We saw improvements had been made following post operative blood clots. Patients were given advice about the risks of blood clots during their pre-assessment appointment. Staff followed up-to-date policies to plan and deliver high quality care according to best practice and national guidance.
