During an inspection looking at part of the service

We undertook a follow up focused inspection of South Cliff Dental Group – Dover on 1 November 2023. This inspection was carried out to review in detail the actions taken by the registered provider to improve the quality of care and to confirm that the practice was now meeting legal requirements.

The inspection was led by 2 CQC inspectors who were supported by a specialist dental adviser.

We undertook a comprehensive inspection of South Cliff Dental Group – Dover on 22 November 2022 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing safe or well led care and was in breach of regulations 12 Safe care and treatment and 17 Good governance of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. A follow up inspection was carried out on 12 April 2023 where ongoing compliance with the regulations 12 Safe care and treatment and 17 Good governance were identified.

We found this practice was providing safe care in accordance with the relevant regulations.

The provider had made improvements in relation to the regulatory breaches we found at our inspection on 12 April 2023.

Background

The provider has 28 practices and this report is about South Cliff Dental Group – Dover.

South Cliff Dental Group – Dover provides NHS and private dental care and treatment for adults and children.

There is level access to the practice for people who use wheelchairs and those with pushchairs. Car parking spaces, including dedicated parking for disabled people, are available near the practice.

The dental team includes 4 dentists, 5 trainee dental nurses, 4 dental therapists, 4 receptionists and a practice manager. The team are supported by staff at head office with HR and compliance. The practice has 4 treatment rooms.

During the inspection we spoke with 1 dentist, 2 trainee dental nurses, 2 dental therapists, 1 receptionist, the practice manager, the compliance manager and the clinical director. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open:

Monday to Saturday from 8:30am to 5:30pm

Sunday from 10:00am to 4:30pm

There were areas where the provider could make improvements. They should:

  • Improve the practice's systems and processes for managing the risks associated with Legionella, taking into account the guidelines issued by the Department of Health in the Health Technical Memorandum 01-05: Decontamination in primary care dental practices, and having regard to The Health and Social Care Act 2008: ‘Code of Practice about the prevention and control of infections and related guidance.’ In particular, ensuring appropriate action is taken when the monthly hot water checks do not reach the desired temperature.
  • Improve the practice's systems for ensuring all clinical staff have adequate immunity for vaccine preventable infectious diseases.

We undertook a follow up focused inspection of South Cliff Dental Group – Dover on 12 April 2023. This inspection was carried out to review the actions taken by the registered provider to improve the quality of care and to confirm that the practice was now meeting legal requirements.

The inspection was led by a CQC inspector who was supported by a specialist dental advisor.

We had previously undertaken a comprehensive inspection of South Cliff Dental Group – Dover on 16 November 2022 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing safe and well-led care and was in breach of regulations 12 Safe care and treatment, 17 Good governance of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

When 1 or more of the 5 questions are not met we require the service to make improvements and send us an action plan. We then inspect again after a reasonable interval, focusing on the area(s) where improvement was required.

The provider had made some changes. However there were new shortfalls identified and continuing regulatory breaches we found at our inspection on 16 November 2022.

We found this practice was not providing well-led care in accordance with the relevant regulations.

The provider had made some changes. However, there were insufficient improvements to put right the shortfalls and they had not responded to some of the regulatory breaches we found at our inspection on 16 November 2022.

The dental team includes 4 dentists, 4 trainee dental nurses, 1 dental hygienist, a practice manager who is also a registered dental nurse and 2 receptionists. The practice has 4 treatment rooms.

During the inspection we spoke with 2 trainee dental nurses and the practice manager. We looked at practice policies, procedures and other records to assess how the service is managed.

The practice is open:

Monday to Friday 8.30am to 5pm

Saturday 9am to 5pm

The practice is closed between 1pm and 2pm for lunch

We identified regulations the provider was not meeting. They must:

  • Ensure care and treatment is provided in a safe way for patients
  • Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care.

There were areas where the provider could make improvements. They should:

  • Implement audits for the prescribing of antibiotic medicines. Taking into account the guidance provided by The College of General Dentistry.

During a routine inspection

We carried out this unannounced comprehensive inspection on 16 November 2022 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission, (CQC), inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment,

we ask the following five questions:

• Is it safe?

• Is it effective?

• Is it caring?

• Is it responsive to people’s needs?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic appeared to be visibly clean and well-maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with medical emergencies. Appropriate medicines and life-saving equipment were available. Although annual training had lapsed.
  • The practice did not have sufficient systems to help them manage risk to patients and staff. In particular fire safety.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The practice had staff recruitment procedures which reflected current legislation.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Patients were treated with dignity and respect and staff took care to protect their privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system took account of patients’ needs.
  • Staff did not feel wholly involved and supported and work as a team.
  • Staff and patients were asked for feedback about the services provided.
  • Complaints were dealt with positively and efficiently in the practice. However, complaints that went to the company head office were not always acknowledged or dealt with in a timely manner.
  • The dental clinic had information governance arrangements.
