At our previous inspection, we said that the provider should review and personalise care plans for patients with long-term conditions. During this assessment, we found a mixed picture. A dedicated long-term conditions nurse ensured that patients received person-centred care and treatment for their long-term conditions. However, while all patients had care plans specific to their needs and conditions, not all were personalised. The service had a plan to focus further on personalisation of patients’ care plans at their next reviews.
