Condition of this registration relating to carrying out this regulated activity
1. The Registered Provider must within 30 days of this condition taking effect undertake a full review and evaluation of their quality assurance systems to ensure effective systems are in place at Ladymead care Home. As part of the review and evaluation, consideration must be made to the following:
a) The quality assurance processes in operation at Ladymead Care Home are fit for purpose and person centered.
b) Staff conducting the quality assurance processes at Ladymead Care Home have the skills and experience to identify shortfalls and act on any improvements required.
c) The registered manager has effective oversight of the clinical decisions being made in relation to risk and service user’s clinical needs at Ladymead Care Home.
2. The Registered Provider must send to the commission within seven days of the review and evaluation being undertaken, a report of their findings in respect of the review and evaluation of the quality assurance systems at Ladymead care Home. Thereafter, there must be ongoing reviews and evaluations of the quality assurance systems at Ladymead Care Home and the findings of the ongoing reviews must be sent to the commission on the last working day of every month after this condition takes effect.
3. The Registered Provider must within 30 days of this condition taking effect
undertake a full review and analysis of the quality assurance processes used to identify risks in relation to infection prevention and control at Ladymead Care Home. This review and analysis must include actions that have been taken to address any shortfalls found in relation to
a) Staff wearing personal protective equipment (PPE) in line with government guidance for COVID-19 infection prevention and control in care homes.
b) The sanitation of Ladymead Care Home during the global pandemic of COVID-19.
c) Testing of residents and staff for COVID-19, to include receiving and acting upon test results in a timely way.
d) Isolation processes at Ladymead Care Home for individual service users who test positive for COVID-19.
e) Processes to manage an outbreak of COVID-19 at Ladymead Care Home.
4. The Registered Provider must send to the commission within seven days of the review and analysis being undertaken, a report of their findings in respect of the review and analysis. Thereafter, there must be ongoing reviews and analysis of the quality assurance processes used at Ladymead care Home to identify risks in relation to infection prevention and control, and the findings of the ongoing reviews must be sent to the commission on the last working day of every month after this condition takes effect
5. The registered provider must within 45 days of this condition taking effect, undertake a full audit of all care plans and risk assessments for service users receiving care at Ladymead Care Home. The audit must include details about how service users’ needs have been assessed and monitored in relation to
a) How service user’s clinical needs are being monitored and met to include the management of seizures, diabetes, pressure care, wound management, fluid monitoring and urinary tract infections.
b) That falls have been monitored and actions taken to help mitigate the risk of future falls
c) That there is clinical oversight and monitoring of staff’s actions when service users have experienced unwitnessed falls or falls that involve service users hitting or injuring their heads to ensure timely, appropriate and safe care has been provided.
d) Medicines are administered in line with the prescriber’s instructions and accurate and contemporaneous medicine records are kept.
6.The Registered Provider must send to the commission within seven days of
the audits being undertaken, a report of their findings in respect of the outcome
of the audits. The report must include any actions identified by the registered
provider to improve care planning in these areas and timescales for
completion. A report of the findings and associated actions from the monthly
audits must be submitted to the CQC on the last working day of every month
after this condition takes effect.
7. The registered provider must within 30 days of this condition taking effect devise and implement an effective system to ensure that the risk and incidence of accidents and incidents at Ladymead Care Home are properly identified and managed. A report of the findings and associated actions from the monthly accident and incident audits must be sent to the CQC on the last working day of every month after this condition takes effect.
Terms of this registration relating to carrying out this regulated activity
The registered provider must ensure that the regulated activity accommodation for persons who require nursing or personal care is managed by an individual who is registered as a manager in respect of that activity at or from all locations.
