This is the first inspection for this newly registered service. This key question has been rated good. The provider and registered manager had established systems to identify safety events and ensure these were investigated thoroughly and any lessons learnt were embedded in to practice. They worked with people and partner organisations to maintain safe systems of care and this was based on openness and honesty. The registered manager and staff worked closely with people to understand what being safe meant to them as well as with external professionals around the best way to achieve this aim. Recruitment practices were meeting requirements. There were enough staff to deliver the care package and systems were in place to ensure new care packages could be supported with adequate numbers of staff. Care packages were only accepted when the registered manager and staff were confident the person’s needs could be met. Medicine management was effective. Staff who administered medicines had the appropriate training. Staff followed appropriate infection control measures. The registered manager ensured staff were trained to use equipment and understand how to check items were fit for purpose.

Learning culture

The provider regularly reviewed the systems and processes in the service to determine if improvements could be made. Action plans were used to identify and monitor where changes were required and how these could improve the service. They used all feedback to assist them improve the quality of care. The registered manager had an effective system in place to monitor the quality and safety of the service and ensure people achieved positive outcomes had been put in place. The registered manager could readily identify where gaps existed and acted swiftly to address them.

People found the care package met their needs. People felt involved in all aspects of the design of the care package and how it was delivered. They found staff were competent, caring and treated people in a dignified manner. A relative said, "The staff are well versed in the training and procedures as well as all health and safety aspects."

The provider critically reviewed the operation of the service and actively made changes as and when these were needed. The registered manager discussed where the service had been and how they prioritised their work to ensure people were safe and well-cared for first. They ensured people were regularly consulted about the support they were offered and received. A staff member said, "If we ever need to raise any concerns these are dealt with in a timely manner."

Safe systems, pathways and transitions

People told us they felt safe and they experienced a smooth transition when the staff took on their care package. Care packages were only accepted when the registered manager and staff were confident the person’s needs could be met. The registered manager ensured care plans contained pertinent information about people’s needs and preferences to enable staff to provide appropriate care.

The provider had a range of policies and procedures to manage risk and monitor the safety of both people and staff. Staff understood when people required support to reduce the risk of avoidable harm, and risk assessments were in place. The registered manager discussed the process for transitioning people to their service and how they worked with individuals prior to accepting and commencing the package of care. They discussed where the service had been and how they prioritised their work to ensure people were safe and well-cared for first. This meant staff could gain a full understanding of the person's needs prior to becoming solely responsible for providing the required support. A staff member said, "Risk assessment are done before we start working with clients and these are regularly reviewed. Care plans are done with the people, their views are taken. They and their families are part of the process." Another staff member said, "We are asked to inform the supervisor/manager if we see any risks whilst we do our daily task so that they can be managed."

The service had been developing good links with the local community so they could work in partnership with other agencies to improve people's opportunities and wellbeing. At present with the limited number of packages professionals reported they had not need to be involved but had no concerns about the service. They found overall the service had the right processes in place.

Systems were in place to ensure people transitioned to the service in a safe manner and there was continuity of care. Over the last year the registered manager had been gradually building the foundations of the operation and only taken care packages when they were confident they could met all aspects of the care and support required. This meant they had reduced the risk of care package break-down. The registered manager ensured care plans contained pertinent information about people’s needs and preferences to enable staff to provide appropriate care. Risk assessments were in place to ensure and assisted staff readily identify how to mitigate risks. The registered manager could readily identify where gaps existed and acted swiftly to address them. Staff said they had training and a good understanding of what to do to make sure people were protected from harm or abuse. Mechanisms were also in place for staff to raise concerns and share their views over the development of the service.

Safeguarding

The registered manager discussed how no safeguarding referrals have been needed but they had ensured staff were provided with training around this aspect of care and their understanding was checked during their supervision session. For the last 5 years the registered manager has been operating a recruitment agency, which is a separate entity, and feel they have learnt a lot from this provision. The registered manager accesses staff from the agency to become permanent members of the team. As a part of the recruitment process they use care provider evaluations for the agency staff and take on staff when this feedback has been very good. A staff member said, "They [the service] are very supportive of me as a staff. Work with them is very collaborative. You can go to them for anything. They are always happy to help, answer questions."

The provider had safeguarding systems in place. Staff said they had training and a good understanding of what to do to make sure people were protected from harm or abuse.

People reported they were very happy with the service. A relative said, "The staff are well versed in the training and procedures as well as all health and safety aspects. They are reliable well presented and go about their tasks efficiently. We have no complaints."

Involving people to manage risks

Staff reported there were enough care workers to meet people's needs and they work together effectively to provide safe care that meets people’s individual needs. They found the provider had systems in place to mitigate the risks of people. Staff understood when people required support to reduce the risk of avoidable harm, and risk assessments were in place. They found the risk assessments effectively assisted them to safely mitigate risks.

People felt involved in all aspects of the design of the care package and how it was delivered. They felt their views were closely listened to and their suggestions acted upon.

Safe environments

People told us staff had the skills and knowledge to carry out their role effectively. They found staff were trained in delivering all the care and support they needed. They were confident staff could carry out their role safely and competently.

Staff told us they have received training around adopting safe working practices and following best practices guidance. They felt safe working at the service and had access to all the appropriate equipment they needed. The registered manager had recently decided to increase staffing, as they were aware the summer holidays would need covering. Staff confirmed they had been involved in providing an induction for new staff and had ensured they understood exactly how to meet people's needs.

The provider ensured staff had the skills and knowledge to carry out their role effectively. Records showed staff had regular supervision and completed a wide range of training which included safeguarding, infection control, medication, moving and handling, and pressure care. There were enough staff to deliver the care packages.

Safe and effective staffing

Staff reported there were enough care workers to meet people's needs and they work together effectively to provide safe care that meets people’s individual needs.

All appropriate recruitment checks were completed and newly appointed care staff went through an induction period. This included training for their role, shadowing an experienced member of staff and having their competencies assessed prior to working independently with people. There were enough staff to deliver the care packages. Records showed there had been no missed calls and staff were always available to deliver the care package at the time requested.

People found the care package met their needs and there were always enough staff to deliver it. A person said, "I have no complaints as the staff always turn up on time and do everything I ask."

Infection prevention and control

The provider had systems in place to mitigate the risks of people and staff from catching and spreading infections.

Staff confirmed they received training on infection control procedures. They had all the equipment they needed to reduce the risk of spreading infections.

People found staff followed expected infection control practices. A person said, "The staff follow the procedures around health and safety."

Medicines optimisation

Score: 3

Staff were trained in medicines management and were assessed as competent to administer people’s medicines. Staff were given clear guidance on when to administer medicines.

People’s medicines were managed in a safe manner. A relative said, “We have had review meetings with the management team regarding new medicines and the charts and were kept up to date in every aspect.”

Assessments were produced, and these fully captured people's needs, and these detailed what medication people received. The registered manager completed regular checks to ensure medicines had been given as prescribed and records were accurate.
