People received personalised care and support specific to their needs and preferences. Care plans reflected people’s health and social care needs. People’s care files enabled staff to support them appropriately in line with their likes, dislikes and preferences.

People’s care, treatment and support promoted equality, removed barriers or delays, and protected their rights. People were constantly encouraged to lead rich and meaningful lives.

Staff told us they supported people to feedback on their care to ensure they were happy with how they were cared for. One said, “We chat with them a lot and try and get across they are free to talk to us about anything and I encourage people to share any concerns and creating a good rapport with them.”

Care records showed that people’s information was personalised to them. Care records were regularly reviewed. The service worked with other health and social care professionals to meet people’s specific needs and identified outcomes.
