We found:

The management of medicines was not safe. The supply of medicines was not consistent. People using the service were not always given medicines as prescribed. Medicine records were not complete and were sometimes amended. Medicine errors were not always reported as incidents.
Staff did not understand safeguarding issues. They did not know how to make a safeguarding referral.
The assessment of people using the service did not identify all of their needs. There was no record that peoples’ views or preferences had been sought.
Most people using the service did not have a care plan.
There were limited records concerning the care, treatment and progress of residents.
There was no permanent manager for the service. The registered manager was not in day to day control or management of the service.
The service was in a period of transition. New systems were being introduced. There had been significant staff changes.

We saw that there were arrangements in place to ensure people's permission was sought to provide care and support and that people had information to help them make informed decisions. We spoke with three people using the service. One person said ''the staff are respectful towards us. They ask our agreement to things; I even signed a contract when I came here. They give you a brochure about what's available and what we are allowed to do.''

There was evidence to show people received safe appropriate personalised care and that their individual needs were established from when they began to use the service. People we spoke with said '' They give us support when we need it and seem to understand how we are feeling about things.''

We were not able to see evidence that staff had received training or had been assessed as competent to safely administer medicines. There was no evidence to show that checks were made to identify if people had any allergies prior to receiving medicines. We saw that there were appropriate arrangements to obtain, store and dispose of medicines.

The provider had systems in place to ensure people's safety and welfare were met by staff who had completed the necessary safety checks prior to their being employed.

Records were found to be fit for purpose and held securely to ensure confidentiality.

People were positive about their time at Ravenswood Road. One person told us he was due to be discharged and was nervous about how he would cope but felt well prepared and supported following his time at Ravenswood. The staff were seen to have a good rapport with each person. People and staff told us people were actively involved in planning their own care and setting their own personal targets. The records seen supported this. The facilities were clean and well maintained. The organised activities people told us about were held regularly and designed to encourage independence. People said staff encourage you to do things on your own.
