Supporting people to live healthier lives

People's care records showed evidence of staff working with other healthcare professionals, such as dietitians, to support people to remain healthy and to ensure people's individual healthcare needs were met.

People told us they were able to access healthcare professionals, such as their GP, when required, and staff supported them to do this. Staff supported people to maximise their independence, where possible. For example, a person using the service was supported by staff to follow exercises set out by the healthcare professional, to improve the person's mobility which had meant the person no longer needed to use mobility aids.

Staff we spoke with demonstrated they knew people well and described how they would recognise when a person's may be unwell, such as a change of communication or behaviour. A staff member said, "All our service users are monitored daily, this can involve observing their presentation. Service users also tell staff if they are not well and staff aim to maintain an open dialogue with them about issues arising."
