Evidence showed people were safe. Risk assessments and management plans were reviewed regularly. Risks related to falls were assessed and managed safely. Equipment was used safely to support people and was checked to ensure it was safe to use. There were systems and processes for staff to follow and incidents and accidents were analysed. Improvements were made in response to learning from incidents. This included people’s bedrooms being set up similarly to their home bedrooms. Following analysis of incidents and accidents the provider introduced staff training and supervisions aimed at continuously improving people's experiences.

We observed people's risks to falls were managed effectively. People had risk management plans and equipment. Records showed incidents of falls were reducing. During the assessment we observed people used fall reduction equipment in line with their care plans. Some people had low beds which made mobility easier. There was meaningful staff engagement with people, and we saw staff used effective distraction techniques. There were enough staff to meet people’s needs. Where people required support, we saw staff were quickly available and anticipated people's needs.

People's risk assessments and management plans were created using people's input and those legally authorised were involved. Equipment such as WIFI sensor mats, bump hats and acoustic monitoring were available and used to manage falls risks. This was in line with current research on managing falls. We saw one new person had their room simulated to their original home bedroom setups. This person told us they liked their room as it was easy for them to find their way around. Support was sought from the GP as well as care home support service which allowed review of medicines and general assessments of people's daily living.

The provider had processes and polices which staff followed. Fall incidents were recorded, investigated and analysed to allow causes to be identified and mitigated. Staff received reflective supervision and learning. The provider had explored environmental aspects of falls risks and introduced trial measures such as a day room for people with high risk of falls.

We saw evidence of staff reflective supervisions and learning. The provider had explored environmental aspects of falls risks and had introduced trial measures such as a day room for people with high risk of falls. New admissions had their rooms simulated to their original home bedroom setups. Evidence showed the falls incidents had reduced in the last three months. Staff told us changes had been made and more learning introduced in response to learning from incidents. They said, “We discuss falls as part of daily care and share updates to ensure falls are reduced”, “The managers have started reflective group supervisions to allow lessons learnt. These are very helpful and allow us to understand how to better support residents” and “Some of our residents wear bump hats and we have seen how effective they have been in reducing injuries.”
