The service was not well- led. There was no registered manager at the service and some people, staff and relatives lacked confidence in senior leaders and did not feel safe raising concerns. This was a breach of regulation 17 (Good Governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

Freedom to speak up

We spoke with 12 members of staff, the majority told us they did not feel able to raise concerns due to fear of intimidation and losing their jobs. They told us when staff had raised concerns, they had lost their jobs or were ignored and nothing was done in response to their concerns. For example, staff had raised concerns about staffing insufficient numbers of staff on shifts, but no action was taken. We spoke with the provider about these concerns shortly after our site visit. The provider commenced an investigation across all their services.

The provider had policies and procedures about raising concerns. There was a confidential whistle blowing helpline number displayed. Staff were aware of how to raise concerns but did not feel confident doing so. We spoke with the provider shortly after our site visit.
