Governance, management and sustainability
The practice had a governance system that included policies and procedures, which were accessible to staff and were reviewed on a regular basis. Systems and processes were embedded. The practice had systems to identify and manage risks. However, the assessment highlighted some additional areas of risk. In particular, oversight of infection control procedures, checking of the medical emergency kit and systems to document incidents and accidents. During the assessment, staff were open to discussion and feedback. After the assessment, evidence was sent to demonstrate areas requiring improvement were acted on immediately. Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations. There were effective processes for undertaking audits and for receiving and acting on safety alerts. We noted a recent incident had been discussed in a staff meeting to ensure learning and prevent re-occurrence, but had not been documented in the practice incident reporting system. We highlighted the importance of using these systems. Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement. Feedback from patients was collected and responded to. Staff demonstrated an open culture in relation to people’s safety. Staff had clear responsibilities, and systems of accountability to support good governance. Staff feedback was obtained through meetings and informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate. The practice recycled waste to improve environmental sustainability.
