• Any alternative medicines discussed
• A discussion of the risks and benefits with the service user
• Any required monitoring and whether it is up to date.
b) The Registered Provider must, within 14 days after this condition takes effect, provide Care Quality Commission with details of the systems put in place in relation to medicine reviews.

a) develop and implement an effective system to identify, review and monitor:
• service users prescribed high risk medicines
• service users with an exacerbation of asthma
• service users with hypothyroidism
• service users with diabetes
• service users who have been prescribed medicines that are the subject of a Medicines and Healthcare products Regulatory Agency (MHRA) alert
b) The Registered Provider must, within 14 days after this condition takes effect, provide Care Quality Commission with details of the systems put in place in relation to identifying, monitoring and reviewing service users.
a) Provide the Care Quality Commission with evidence that the backlog of service user notes requiring summarisation has been cleared and that those service user notes are up to date, accurate and have been reviewed by suitably competent and skilled staff,
b) Develop and implement an effective system to ensure summarisation of notes, for service users that have been transferred to the practice, is completed on a timely basis by suitably competent and skilled staff.
c) Provide Care Quality Commission with details of the systems put in place in relation to summarisation of notes.
