Staff worked with patients to look at ways of managing their symptoms and to create innovative coping strategies. Staff completed appropriate physical health monitoring and had access to a fully equipped physical health care suite. The trust had appropriate systems and processes in place to ensure they were delivering evidence- based care and treatment. They had regular medicines and therapies teams meetings where they reviewed the medicines and treatment plans in place for patients. The trust also had a National Institute for Health and Care Excellence (NICE) policy which gave staff guidance on current best practice guidance.

Delivering evidence-based care and treatment

Staff worked with patients to look at ways of managing their symptoms and to create innovative coping strategies. We were told 1 patient who was experiencing auditory hallucinations which were intrusive and caused her to shout at them had learnt to use her mobile phone whilst out in public to respond to the voices in an accepted way.

Staff ensured that physical health care plans were in place where appropriate and updated where necessary. We saw patients had been referred to specialist services such as urology and diabetic services. Staff had completed the appropriate physical health monitoring following rapid tranquilisation and had a fully equipped physical health suite. We reviewed 4 National Early Warning Score (NEWS) forms all of which had been completed fully and staff had acted according to the severity of the score.

The service had monthly medicines optimisations meetings in place. These were attended by clinicians, therapy staff and pharmacists and looked if treatment plans followed best practice and guidance in place. The meetings recorded clear actions, that were reviewed at each meeting. The trust also had a National Institute for Health and Care Excellence (NICE) policy in place this looked at The National Institute for Health and Care Excellence guidance and regulations including those within the Health and Social Care Act ensuring the trust is delivering evidence -based care.
