The provider was in the process of reviewing and improving daily records of people’s care to demonstrate people’s care was being delivered in line with best practice guidance and nationally recognised tools. We received positive feedback on the quality and quantity of food and drinks provided. The service monitored people’s weights and risks associated with nutrition and hydration were assessed.

Delivering evidence-based care and treatment

We received some feedback regarding continence care, that people were not always supported to go to the toilet as often as they needed, despite this being a part of their planned care. This impacted on people's dignity. People told us they enjoyed the meals provided at the service and had enough to eat and drink. One relative told us, “Happy with the meals, gets a choice, snacks available, never hungry, quality of food is good, has put on weight which was a good thing.”

Staff explained there were sometimes inconsistencies in the recording of daily care. Staff told us they were aware of care either given, or not given, on specific days however daily records did not always accurately reflect this. Staff were aware of people who were considered high risk of malnutrition or dehydration and ways to support them to improve their intake, such as fortifying foods to increase calorie intake. Leaders were aware of the challenges with the format of daily records and had imminent plans to replace the form. Following our assessment the registered manager updated us and felt this change had been positive. Leaders reflected on feedback received about the quality and quantity of the food provided at the service and were working to improve the meals provided with support from the local authority.

Daily care records were not always fully completed to show people had received the care and support they required. Nationally recognised tools were used to inform people’s care. For example, a tool used to monitor the risk of malnutrition. People’s weights were regularly monitored, and we saw evidence of referrals to dietitian and other relevant professionals where people had lost weight.
