Effective -This key question has been rated Good. This meant people's outcomes were consistently good, and people's feedback confirmed this. People's needs were assessed, and care was delivered in line with current standards to achieve effective outcomes. The provider ensured the service worked effectively within and across organisations to deliver effective care, support and treatment. Mental capacity assessments and best interest decisions were completed in line with best practice.

Assessing needs

People confirmed they were involved in planning their care. People told us they felt staff listened to them and discussed their care with them.

The acting manager told us how they ensured people’s needs were regularly assessed and how they involved people and their relatives in reviews of people’s care.

The support people required was assessed prior to admission to ensure their needs could be met at the home. Ongoing reviews ensured any changes were identified and care plans were updated.

How staff, teams and services work together

Score: 3

People were supported by staff that worked well together and with external professionals to provide holistic care. For example, people were supported to access health professionals such as doctors, dentists, opticians and chiropodists. One person told us, “It’s all organised for us, if we need to see a doctor the staff sort it out.” Another person told us, “The staff have been very good at ensuring I see the doctor if I need to.”

The acting manager told us of the positive working relationships they had built with external professionals to ensure people received support to meet their needs.

External professionals confirmed they worked with the service to ensure people received the support they needed .

We observed staff to have access to people’s care records to support their delivery of care.

Monitoring and improving outcomes

People told us staff involved them in their care, and they made choices which staff supported and encouraged.

The acting manager told us how they ensured people’s needs were regularly assessed and how they involved people, their relatives and external professionals in reviews of people’s needs.

Staff used the systems in place to ensure people’s health needs were monitored in line with their care plans.

The provider worked in line with the Mental Capacity Act. Mental capacity assessments and best interest decisions were in place and had been completed in line with best practice. Where people were deprived of their liberty, Deprivation of Liberty Safeguards were in place and people were supported in line with their agreed plans.
