Our view of the service

Ryhope Manor Care Home provides accommodation with nursing and personal care for up to 31 people. At the time of the assessment there were 30 people using the service. Most of the people at the service were living with dementia. We commenced the assessment because we had received concerns regarding people’s care and support. This was an unannounced assessment, which meant the provider was not told an assessment was going to be starting beforehand. We carried out 4 site visits to the home on 2, 6, 11 and 18 September 2024. During the assessment we looked at all 33 quality statements. We found 8 breaches of the regulations in relation to person centred care, safe care and treatment, safeguarding people from abuse and improper treatment, staffing, receiving and acting on complaints, fit and proper persons employed, good governance and duty of candour. The environment and equipment were not always clean, well maintained or serviced in line with legal requirements. Risks relating to people’s care and support had not been fully assessed and medicines were not managed safely. There was a lack of evidence to demonstrate staff were suitably trained and skilled. The provider’s recruitment policy was not detailed and did not fully reflect recruitment practices at the home. An effective quality monitoring system was not in place to ensure regulatory requirements were met. Following our feedback, management staff took action to address some of the issues identified and formulated an action plan to address the outstanding areas. The service has been placed into special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.

People's experience of this service

People and relatives told us people felt safe; we found however, that an effective safeguarding system to ensure people were protected from the risk of abuse was not in place. This placed people at risk of harm. We received feedback from several people and relatives that more activities to meet people’s social and emotional needs were required. This was confirmed by our own observations. Following our feedback; activities provision was increased from 3 to 5 days a week. Whilst people and relatives told us that staff supported people to access health and social care services; we found an effective system was not fully in place to ensure risks relating to people’s health and wellbeing were identified and actioned. We identified shortfalls relating to people’s specific medical conditions, meeting people’s social needs, pressure area care, medicines management, the use of specialist equipment and nutrition which posed a risk to people’s health and wellbeing. People and relatives generally spoke positively about meals at the home. We found however, the home’s menus did not demonstrate how people were being offered the government’s recommended 5 portions of fruit and vegetables a day. In addition, there was no separate menu for people who required a modified textured diet. Staff explained that sometimes people who required this type of diet received repetitive meals. Whilst people told us they were treated with kindness; we found the condition of the environment, including the outside space and cleanliness did not fully demonstrate that the provider fully respected people's privacy, dignity and wellbeing.
