Safe
People were protected from the risk of harm and abuse. People were supported to understand and manage risk. Support plans were clear and provided sufficient guidance to staff to keep people safe. There were sufficient and appropriately trained staff in place to support people.

Learning culture
Lessons learnt were shared in team meetings. We reviewed some recent meeting minutes which demonstrated how staff discussed and learnt from the incidents.
The manager understood the requirements of the duty of candour. This is their duty to be honest and open about any accident or incident that had caused or placed a person at risk of harm. The provider understood CQC requirements, and understood to inform the local safeguarding team, of incidents including potential safeguarding issues, disruption to the service and serious injury.
People told us the office communicated with them if there were changes to the times of their scheduled care visits. One relative stated, “The office phoned to let me know, and offered a visit later in the day but after 9am does not work for us. They’ve gotten better about letting us know.”

Safeguarding

Involving people to manage risks
Accident and incidents were recording using an electronic monitoring system. Appropriate actions were taken to keep people safe.
People’s care plans and risk assessments included details of how to support them to stay well by providing care to safely manage their skin integrity, eating, drinking and mobility.
The manager told us all care plans are being reviewed and stated, “We are improving our communication and changing the culture of the service so we work collaboratively to achieve the best outcomes for our customers.”
