At our last inspection the provider had failed to ensure effective governance arrangements were in place and their systems and processes to monitor the quality and safety of the service were ineffective. At this inspection improvements had been made and the service was no longer in breach of regulation 17. The provider had implemented a range of effective quality audits and checks which focused on improving outcomes for people. This demonstrated lessons had been learned. Staff felt supported and were confident to raise any concerns or suggest areas for improvement. Staff felt listened to and described the management team as approachable. Staff told us there was a strong emphasis on teamwork.

Capable, compassionate and inclusive leaders

Staff were positive about the changes made by the provider at St Andrew's House. Staff were confident that the new systems and processes supported them to support people safely. Staff had the information they needed to raise any concerns to the management team and were confident these would be acted on. Staff showed commitment to providing good care.

The provider had implemented a range of effective quality audits and checks which focused on improving outcomes for people. For example, spot checks on staff were completed to ensure they supported people in line with the provider's values. Improvements had been made, by the provider, to ensure staff understood their role and responsibilities, this gave clear direction to staff. One member of staff told us, "This has been really helpful and everyone knows what they are doing and what needs to be done." This demonstrated lessons had been learned. Staff felt supported and were confident to raise any concerns or suggest areas for improvement. Staff felt listened to and valued. Staff told us there was a strong emphasis on teamwork.

Freedom to speak up

Staff told us the provider had a culture of speaking up about concerns. This had been achieved through the management team involving all staff in flash meetings and encouraging contributions from all those attending, building staff confidences to speak up. One staff member told us, "I feel confident to speak up and feel my opinions are valued." Another staff member said, "We have pulled together to problem-solve and offer ideas, it's been great to see and for the team to work together to improve the service." The management team had shared information about where improvements had been needed with staff and teamwork had led to those improvements being achieved.

Governance, management and sustainability

Score: 3

Staff told us they were aware of the checks in place to ensure safe care was provided. Care staff felt there was good communication between the management team and themselves. Care staff felt the management team were supportive and accessible to them, either within the home or on-call.

Systems were in place to ensure good governance and oversight by the provider. This included audits of medication records and medicine balances, infection control and health and safety checks. The provider ensured CQC and local authority were informed of specific incidents that occurred as legally required.

Learning, improvement and innovation

Score: 3

Staff had been concerned about the future of the care home, but had been reassured by the commitment shown by the provider and management team. One staff member told us, "Changes in the management team have been positive and made the home stronger." Another staff member said, "There have been lots of changes for the better, it has been hard work, but we are going the right way."

Lessons had been learned from the last inspection and improvements had been made. Systems and processes had been implemented to ensure effective managerial oversight took place. However, time was still needed to sustain and embed the new systems and processes. The provider continued to work toward achieving their service improvement plan and demonstrated commitment for further improvements.
