At our last inspection we rated this key question as good. At this inspection the rating has remained the same. This meant people’s outcomes were consistently good, and people’s feedback confirmed this. The service was effective and used the correct processes and assessments to achieve this. Care and treatment were evidence based and focused on person centred care. People’s care records were written to a good standard and monitored to enable continuous improvement.

People were supported to access translation services and signposted to external resources to help capture their voice, needs and preferences. Printed patient information and appropriate signposting to external resources were available in multiple formats to meet individual needs.

People knew how to access weekend and out of hours services and were signposted to services such as The Crisis Team and The Samaritans by staff who provided printed contact details so people could easily access them.

We looked at 8 care records that were written to a good standard, however, we saw in one case one person identified as at risk of self harm had not answered follow up calls. Staff did not record what actions they had taken; however, we were assured by the service that people were contacted by staff at the time.

Assessing needs

People said that staff provided them with care and support enabling them to live as they want to, seeing them as a unique person with skills, strengths and goals. They felt involved in their care planning and risk assessments and received care that is co-ordinated. People said they felt that everyone within the team worked well together, and staff were open and honest about their care options.

Staff said that they always encouraged people to be as involved as possible with both the care planning and risk assessment process. Staff demonstrated that they understood the value of the people’s voice as this maintained a good quality of person-centred care and support relevant to the individual.

Referrals were processed in a timely manner and people are screened to ensure that they receive the most efficient and appropriate care.
