Safe
At our last inspection we rated this key question as good. At this inspection the rating has remained the same, rating as good. This meant people were safe and protected from avoidable harm. This service maximised the effectiveness of people’s care and treatment by assessing and reviewing their health, care, well being and communication needs with them, the service provided care and treatment in a way which made people feel safe, supported, and listened to. People who we spoke to said they were treated with dignity and respect and were supported to make choices that balance risks of harm with positive choices about their lives. People were encouraged to take part in their care planning, and risk assessments about their care. these were person centred, proportionate, and regularly reviewed with the person.

Involving people to manage risks
People said that they felt safe and supported to understand and manage risks while in the community. People said that they were able to contact their nurse or a member of team if they felt their physical or mental health became worse. People said they were reassured that staff would be available to help and were aware of the processes in place to get support out of hours and during the weekend.
Staff said that people were informed about any risks and how to keep themselves safe, with clear processes in place for people to contact them between appointments when required. Staff helped people to understand their symptoms and recognise when they might need more support. When people communicated their needs, emotions or distress, staff were able to manage this in a positive way.
Staff said that they had a person-centred approach and always attempted to involve people and family/carers in their care. When appropriate the people and family/carers were encouraged to have input in completing and reviewing both risk assessments and care plans to ensure that the peoples voice was heard.
Risk assessments were observed to be completed with people’s input towards their care.
